Spondylolisthesis is a sign that the spine is unstable. It occurs when degeneration weakens the connections between the vertebrae. Loosening in the area of the joints and tendons especially can cause the vertebrae to slip. This instability manifests itself as back pain, which gets worse under strain.

Pain at night, e.g. when turning over in bed, are indicators of major instability. The body attempts to compensate the loss of stability by depositing bone (spondylosis), enlarging the joints (spondylarthrosis) and thickening ligament structures (Ligamentum flavum hypertrophy), which also causes narrowing of the spinal canal (known as spinal stenosis) with pressure on the nerve structures. In younger patients, the cause can be a congenital weakness or loosening of the isthmus (spondylolysis).

In a few rare cases, the upper disc slides a long way forward. In addition to back pain, over-extension of the nerves can cause leg pain and even dysfunction.

Conservative treatment is possible with moderate symptoms of instability.

Surgical treatment is carried out when:

  • Conservative treatment has failed
  • There is increasing muscle weakness
  • The pain cannot be controlled with medications

Established surgical methods include:

  • Fusion surgery with decompression
